Does my insurance cover an eye exam?
Your insurance coverage is depends on your provider and coverage type. You can contact the customer service number on the back of your insurance card and ask the agent if "routine" or "comprehensive" eye exams are covered and, if so, what exactly the coverage is.
If you prefer, the staff at Provision Eyecare can contact your insurance provider on your behalf. In order for us to do this, we will need your insurance provider and insurance ID number, the subscriber's name and date of birth, and the patient's name and date of birth.

Why am I being charged more for a contact lens exam than for a routine eye exam?
You may notice an increase in charge when coming in for your Contact Lens Exam as opposed to our routine Eye Exam. This is because a contact lens prescription requires all of the testing that's done in an annual routine eye exam plus additional testing that must be charged out separately to comply with vision insurance charges. Simply put, a Contact Lens Exam is a Routine Eye Exam on steroids, therefore, it is of an additional cost.
If you wear contacts, you are required to have an annual Contact Lens Exam in order to maintain your prescription (even if your prescription isn’t changing).

What if my insurance is out of network?
Don't see your insurance provider on our website? Unfortunately, this most likely means that it is out of our network, however, we make it possible (and easy!) for you to still be a patient of Provision Eyecare.
If your insurance is out of network, you will pay cash the same day as your service. This upfront cost will be discounted because we won't be going through insurance (discounted amount to be decided day of visit).
After paying out of pocket, it is possible that your insurance provider will reimburse you for a portion of the cost. There are no guarantees on full reimbursement from your insurance provider, in fact, it typically isn’t the full amount. In order to apply for reimbursement, you will need to fill out out-of-network paperwork, which we can provide and assist you in getting it completed and submitted.
